Bearing Hub Cap Dislocation

I trail a relatively short distance (less than 10 miles) and often one of my Hub-Caps, always on the same wheel, comes off somewhere on the road. This is a tandem trailer and I have not been able to figure what the source of the problem is. I have replaced all 8 wheel bearings but it still happens. Can you give me a path to follow?

It may sound strange, but the first thing to check to make is the cotter pin. It needs to be bent pretty tight to the bolt.

Seen many a cotter pin walk a wheel cap right off

If the caps are the metal press-in types you can trying squeezing them in a vise to just slightly deform the outer circumference of the caps to provide a bit of "interference fit" when you tap them back in place....
