Cylinder sleeve rotated

I have a 1997 225hp 3.0L carbureted Mariner that was recently rebuilt. Since the rebuild cylinder 2 was not firing properly and it turns out that the cylinder sleeve had rotated causing blockage of the ports. The rebuilders have said they will fix it but what a pain to remove my powerhead and ship it to them AGAIN! Any ideas what causes this and should I be concerned that it will happen again?

Bad sleeve or machining, to sleeve a block most people use diamond ip die graind to split sleeve and it will slide right out, to install blocks needs heated and sleeve dropped in with loctite sleeve locker(green) and then machined flat. I would not worry about it unless that cylinder was welded due to damage and then resleeved as it may be machined with excessive clearance.

The bullet is right on the money. As I was calling around trying to find some one to bore my block that was one of the problems they said they might encounter.
