89 4.3 cobra stuck in gear (with new cables)

OK so I replaced the lower shift cable and did all the adjustments as per stuarts page. everything seemed to be working great but then I got stuck in reverse gear, really stuck, i had to remove the drive and sure enough the shift lever had popped out of the plastic cable guide. What would cause this? there doesn't seem to be anything to keep it from happening again. help?

Update: had a good look at the shift cable and found that it may not be secured properly in the leg. The nut on the outside of the leg housing is tight but when I wiggle the cable I can see the plastic guide in the leg move. Seems me that should be tighter. Is there a nut on the inside? I can't remember, and can't see inside without fully removing the leg.
