I have a 2005 Mercruiser, and Its leaking water out of this hole out the back of the boat. Is this normal? Also my exhaust Bellows is cracking. Is this a major concern?

The leaking exhaust boot is not an issue other then an indication that maybe to other boots need to be replaced due to time.

As for the leak, if that is on the transom plate (appears to be) then that is an issue

I could be wrong but believe those are exhaust ports also. Its where my water comes out after circulating though the engine. I don't have the rust though.

Agree, those pics help show they are the exhaust relief ports, so no issue

A close up pic is good, but need one further away to get a Reference of where the spot is

On your rubber boots between the outdrive and it's mounting bracket, over time the material will become brittle and when extended (after sitting for a period in storage) it can/will crack open. The purpose of those boots to the outdrive are to prevent water intrusion into the boat and the working parts therein. With the exhaust boot usually below the water line at rest, your boat can fill with water if cracked!

Thanks for the responses guys. So do I need to replace the exhaust bellows soon then? Or is it ok for a trip to the lake?

The exhaust boot doesn't even need to be there, water that enters into it stays in the exhaust tube. The boot above it (U joint) will let water in if its cracked. I would recommend replacing all boots including the shift boot because the when one starts to go, the others are not far behind.

The exhaust relief ports are to reduce back pressure from the exhaust when idling, they are lower then the exhaust boot

Not the exhaust bellows. It's filled with water at rest, cracked or not. Cracked drive shaft bellows can let water in.

Just out of curiosity what is the purpose of the exhaust relief ports?

Reduced exhaust back pressure at low RPMs.

A word of caution, and maybe advice.

Sounds like the boat has been sitting for a while. Before loading up and heading to the lake for a fun day, it would be advisable to do the annual maintenance recommended for that drive and motor. Doing the maintenance would give you a much better chance of having a fun day instead of a bad day.

Maintenance on a sterndrive should include removing the outdrive - a 10 minute job to remove. Draining and inspecting the oil that comes out of the sterndrive. Inspecting the condition of the rubber boots from the inside where you can see if any cracks are present and water was present the last time it was run. Greasing the u-joints and inspecting them for operability. Inspecting the gimbal bearing and in some, greasing it. Gimbal bearings should be smooth as silk. Check engine alignment and reinstall and refill outdrive. Attach water muffs and check that everything operates properly.

That's the minimum annual servicing. There is pretty much no one on these forums who doesn't do that maintenance on their sterndrives. Yeah, one can throw the dice and skip this one year and usually get away with it if everything was perfect and maintenance was done well last year. But a parked boat is asking for trouble on the water and can turn a fun day into a dangerous nightmare. Too easy to have something stop working at just the wrong moment. And you can't just pull over to the side and call a tow truck.

My 2¢.

Sounds ready to go. Good deal. At least the cracked exhaust bellows is a non-issue. It is underwater inside and out.

Happy boating.
