Funny noise-help

I have been hearing a funny like sucking noise coming out of the rear of my 89 evinrude 40hp motor. I first thought it was water coming up into the exhaust port on back of motor in rough water but now I notice it on calm water. It doesn't happen all the time but I'm stumped.

Is the motor pumping water, i.e. water coming from the tell tale?

Yes plenty of water from tell-tell. I have noticed water splahing up into the exhaust port on choppy days and making this noise so thought this is whats wrong but also does it on calm days.
