Are you looking for clearance from the top of the hook to the first rung of the ladder, or in the width of the hook itself?
If width of the hook is the issue, you might see if a local metal shop can fabricate something for you out of stainless. Or, they may be able to take an existing ladder, and modify it to give you the extra clearance you need.
