How bad is it?

The pics tell it all. Some bone head put the clamp in the wrong place. Wore thru boot. Both bearing totally gone cept for a few balls, and a c clip. Every thing else gone. And it still ran with only a slight rattle.
How bad is it and what needs done?

You need a new flywheel cover. The portion of the outer flange that is missing is what protects the bellows from the ujoints when trimmed up and when turning.
Replace the pilot bearing in the crankshaft too while you have it apart.

Replacing the bellows every other year and paying attention to ANY strange noises, prevents that kind of mess.

I agree. I just bought the boat last week. When i got it fired up heard the rattle I took it apart. What a mess.
Can it be replaced with out removing the lower unit or motor?

The motor will have to come out. The rest of the outdrive can stay in place.

There's a good chance that you'll have pieces of metal and metal shavings from the bearings, races, and the flywheel cover trapped in the flywheel cover. Might even want to pull off the flywheel and starter and clean in and behind those as well.

If it's any consolation, you're not the first one to have this happen. When I bought my boat, the guy told me that all it needed was a bellows! You've got lots of help here. Just be sure to add enough information when you ask questions so people know what engine you have. Pictures are a great help.
