Johnson 1998 50hp trim leak

I just changed my tilt/trim motor 3 months ago, work great. The hydraulic fluid leaks out of the top of the cylinder around the piston that raises the engine. I see Johnson makes an o-ring kit for this unit, but I would like to make sure thats the right product:

Also the hydraulic cylinder has 4 holes on top which looks like it takes a proprietary tool, does anyone know what the name of it is? Any help or info would be great, thanks.
