Are you sure that the lobes of the dog or gear are not slightly rounded? They should be precisely machined with right angle edges.
Usually when it jumps out of gear during higher rpms, the shift cable will keep tension on the shift linkages, pulling it back into gear and getting the standard THUNK like you are hitting a log. Since yours does not, I am going to guess that you're improperly adjusted or you have worn shift linkages, along with slightly rounded lobes on either the dog or forward gear or both. But is just a guess.
