Start simple verify shifter cables are installed properly. Change lower unit oil if it is milky then you have alower unit problem.

Clicking or ratcheting sound? Which one? Gearboxes are designed to engage quickly. If you try to ease them in you're going to cause problems. There are several things which could cause your problem. First as suggested, check your cable adjustment. Does it start in gear?If it does, thats definately a linkage adjustment. If you have to drop the lower unit there are a few things you can verify before having to tear into it. Look at your shift-shaft connector. Are the inner splines in good shape? Does the gearbox shift manually with no problems? Is this a problem that just started occuring, or has it gradually worked its way into what its doing?

Has the boat set in the water for long periods as these had a mild steel shift shaft which will rotten and twist throwing shifting out of wack...
