Outboards regularly fool people who think they "run great" out of the water with no load or exhaust back pressure on them.
Your engine does not run great. It just sounds like it does when it has no work to do. I think it may have several problems.
Step one: Do a compression check. All cylinders should be over about 90psi and within 10% of each other.
Step two: Crank her up on the muffs and warm her up at idle. Then disable each spark plug, one at a time. If the idle drops or the engine quits that cylinder is making power, if the idle does not change that is a dead cylinder.
If there are dead cylinders in one bank only, you have an ignition problem. If you have dead cylinders side by side you have a carburetor problem.
You didn't mention whether you put a new impeller in her. If not, do that now.
Let us know the results of the above tests. We will use that info to suggest what to do next.
