You put an adapter/attachment thingee into the hole and attach a garden hose,
the other end is connected to a garden type tap,
you then turn on said tap and water flows through the motor case flushing out salt "crystals " etc etc that may otherwise block the cooling passages in said motor,
it's part of normal maintenace,
you should try and find a operators handbook BEFORE running this outboard,
it will also tell you whether this particular motor needs to be running during the above operation,
some don't,
but l think it would be wise if you did it anyway,
