Top hole is not used. Bottom hole goes into oil pressure gallery. On my mercruiser this has a T for the oil pressure switch and the oil pressure sender. I think it’s 1/8” npt, if not that it’s 1/4”npt. If you don’t have any senders or switches to go in just plug it with a pipe plug
As bondo states holding the gasket and cover vertical may be a pain.
You could either put bolts in it to hold the gasket or glue the gasket to the cover with RTV or gasket adhesive, let that set up then put it on.
You can always try it with the dizzy in place and take it out only if needed...

easiest way to tell is crack your gas cap open if it is under vacuum then your vent is plugged, and if you open it it will remove the restriction getting fuel out of a tank under vacuum
yes its to heat under the carb for atomization as the vortec head has no exhaust crossover. I am not 100% sure if it takes exhaust gas or water- but thought it was exhaust not water. Definitely doesnt not circulate, assume it gets warm by being connected pathway.

What is the gap on the plugs looks fairly wide but hard to tell from the screen?
I would make sure the filter is clean, it would be the first thought when more air gets it running, but that seems to not be required every time?

it at least bears looking into especially given the issues known with the crab style caps. in my experience if you have a good spark and any gas presence it will fire off easily.
I would think needing to add throttle would be indicative of the IAC not working or screen being clogged.
I wouldn't chase the rabbit hole of fuel pressure based on what you outline

you're right, this isn't a repair sleeve.
it does go on differently - this is the bearing buddy version. Not sure if this better handles pressurized grease like with a bearing buddy ? Still not clear how the sleeve stays stationary on the spindle...
